Patients’ Rights and Responsibilities
You have the right to:

- - Receive information necessary to consent or refuse proposed treatment, including known risks, alternatives, and cost implications.
You are responsible for:
• Contact the clinic at least 24 hours in advance if you cannot keep an appointment.
• Provide complete and correct information to your health-care team and notify them of any changes in your health.
• Let staff and clinicians know if you do not understand any of the information given to you or if you have concerns.
• Treat clinic staff, patients, interns, and supervisors with consideration and respect. Any form of violence is unacceptable, which includes verbal or physical abuse or threatening behaviour.
Knowing and understanding your rights and responsibilities will make your relationship with our clinics mutually beneficial.
Cancellation Policy
Less than 24 hours’ notice or missed appointments are subject to a cancellation fee of between $25 and $45, depending on the service. See individual Clinic Rate pages for specific cancellation details.
Fragrance-free Facility
CCNM clinics are fragrance-free areas. Please do not wear any product that smells strong enough to be noticed by others, such as perfume, aftershave, body lotion and hair products. Read the full Fragrance-free Policy.
